There are many issues which need to be addressed even before you have been discharged home from hospital or a long-term care facility.   We work with you and your rehabilitation team to see that housing accessibility and transportation issues are addressed and that personal attendant care services are provided.  We help you access the financial resources available such as insurance benefits, non-profit agency and OHIP funded services to ensure that the transition occurs smoothly.
